Wherever possible, please provide a phone number, email address, or mailing address that may already be connected to our system, to help us verify your identity. Depending on the nature of the information sought, we may require a copy of an official form of photo identification to verify your identity.
While in most cases we will be happy to provide you with copies of the information you request free of charge, we nevertheless reserve the right, in accordance with Article 12 of the GDPR to charge a fee or refuse the request if it is considered to be “manifestly unfounded or excessive”. However, we will make every effort to provide you with a satisfactory form of access or summary of information if suitable.
Please note: The details you provide will only be used for the express purpose of making a GDPR Data Access Request. We will provide you with a response within 30 days of receiving a completed form and enough information to verify your identity and process your request. If we are unable to process your request within this timeframe, we may extend this period by two months, and will contact you with an explanation about the delay.
